The procedure and criteria of medical examination of seafarers are prescribed by the Cabinet Regulation No. 273 “Regulations regarding medical fitness of seafarers for work on a ship” adopted 3 June 2014.

Upon taking the medical examination, the applicant shall take along a valid personal identification document (passport, ID card or seaman’s discharge book) and present it to the medical practitioner of seafarers upon request.

Before commencement of the medical examination the seafarer shall fill in the personal declaration of the seafarer’s health examination form giving true information about his/her health condition and deliver the previously issued medical certificate to the medical practitioner of seafarers.

During validity of his/her medical certificate a seafarer must inform the medical practitioner within 15 days from the occurrence of the following:

  1. a seafarer has suffered more than 30 days of incapacitation;
  2. a seafarer has been repatriated from the ship due to medical reasons;
  3. a seafarer is aware that he/she is no longer medically fit for duty; or
  4. his/her health condition may endanger health or safety of other persons on board.

A seafarer must retain the original of his/her medical certificate as evidence of medical fitness while serving on-board ship.
