The procedure and criteria of medical examination of seafarers are prescribed by the Cabinet Regulation No. 273 “Regulations regarding medical fitness of seafarers for work on a ship” adopted 3 June 2014.
Upon taking the medical examination, the applicant shall take along a valid personal identification document (passport, ID card or seaman’s discharge book) and present it to the medical practitioner of seafarers upon request. |
Before commencement of the medical examination the seafarer shall fill in the personal declaration of the seafarer’s health examination form giving true information about his/her health condition and deliver the previously issued medical certificate to the medical practitioner of seafarers.
At the beginning of the seafarer’s health examination form a correct department (deck/engine/radio/catering/other) the seafarer intends to work in shall be indicated.
During validity of his/her medical certificate a seafarer must inform the medical practitioner within 15 days from the occurrence of the following:
- a seafarer has suffered more than 30 days of incapacitation;
- a seafarer has been repatriated from the ship due to medical reasons;
- a seafarer is aware that he/she is no longer medically fit for duty; or
- his/her health condition may endanger health or safety of other persons on board.
A seafarer must retain the original of his/her medical certificate as evidence of medical fitness while serving on-board ship.
Other relevant information can be found on the reverse side of the medical certificate (Annex 1 to the Regulation No. 273).
